🧠 The Habit of Systems


Most people think systems slow them down.


You and I know the truth: a well‑built system gives time back.


I’ve always been a creature of habit.


And yes, I’m one of those people who will write a task down just to cross it off.

(Go ahead and roll your eyes — I’ll be over here happily checking boxes.)


Before EMRs, my “brain sheet” was my lifeline.

Neat boxes. Clean lines. Everything in its place.

·       Patient name, age, diagnosis in the top left

·       Diet and feeding assist in the top right

·       Meds by time with checkmarks for completion

·       Cares — dressing changes, vitals, treatments — lined up and ready to go


It wasn’t fancy. It wasn’t digital.

But it kept residents safe because it kept me organized.


That same mindset applies beautifully to Resident Safety in the OIG’s Nursing Facility ISPG.

The five areas are clear, structured, and absolutely checklist‑worthy:

·       Resident Abuse and Neglect

·       Staff Screening

·       Emergency Preparedness and Life Safety

·       Infection Control

·       Facility‑Initiated Discharges


CMS has already defined the Requirements for Participation of each one.


Facilities don’t need to reinvent the wheel — they just need a system to routinely evaluate whether their processes actually meet the ROPs.


A checklist isn’t bureaucracy.

It’s accountability.

It’s clarity.

It’s safety.


🔧 How have you systematized your work

Everyone has a “brain sheet,” even if they don’t call it that.

Some are intentional.

Some are accidental.

Some are… chaos with a clipboard.
